又到年度馬拉松盛事,跑步健兒及愛好者都在努力備戰當中。熟練的跑者們或有聽過其中一個能夠提升跑步效率的「送髖」技術,增加步幅、速度,同時能省力,讓跑手跑得更快更遠。若你有在練習這個技巧,但發現一直未能掌握到,甚至在練習後大髀罅有痛楚,可能是因為患上「股骨髖臼撞擊症」。
髖關節是由股骨及盤骨的髖臼組成,股骨的頭呈球狀,與髖臼組成一個球窩關節,讓髖關節能夠有大幅度的活動度。但有時候因為某些原因,這個關節對應不順,如股骨頭不夠圓、接合的位置不正確,又或是髖臼窩太深或方向錯誤等,在患者進行大幅度動作時就會引起磨擦和碰撞,輕則活動受限,重則引致軟骨磨損及髖臼唇撕裂。如長久不正視問題,或會令跑步生涯縮短,甚至引起退化性關節炎,連走路都成問題。
如症狀輕微,可請教跑步教練改變跑姿避免引起撞擊、進行伸展及強化運動以改善問題。但如症狀持續三個月以上,運動表現依然受限,便可能需要考慮手術方案。近年開始愈來愈多人採用髖關節鏡這項微創手術,只需在大脾外側開三至四個約1至2厘米小傷口,便可解決上面提及的大部分問題,患者術後需要以拐杖輔助幾星期,然後通常可在三個月後恢復訓練。值得一提的是,若日常生活也有痛楚及活動受限,髖關節可能已經開始退化,進行換骹手術才是更長遠方案。
