港大醫學院自2018年起在瑪麗醫院進行機械臂髖關節置換手術,從累積了五年的經驗所見,此手術對治療較常見的髖關節疾病(如退化性關節炎及髖關節股骨頭壞死等)固然絕無問題;而對於複雜的病例則更為有效。如先天性髖關節發育不良,及創傷後的退化性關節炎的患者,大多曾接受內固定手術;又例如某些強直性脊椎炎患者,其髖關節已完全融合並失去活動能力。上述各種複雜的病情,均會增加髖關節置換手術的難度。過往傳統的手術一般需要四至五小時方可完成,但在機械臂的輔助下,手術時間可縮減至兩至三小時左右。
機械臂能夠在髖關節置換手術的各個階段,協助醫生更精準和有效地施行手術,提升手術的治療效果。術前在機械臂的輔助下,醫生可以準確地量度病人骨骼尺寸,並找出放置臼杯最合適的位置,提供準確的術前電腦掃描計劃。手術過程中,機械臂可協助醫生控制方向,以及準確偵測磨骨的深度,當達到合適的深度便會自動停止;比起傳統手術醫生需要一邊磨骨一邊照X光,並以人手感覺磨骨深度更為準確,減少手術帶來的創傷。全髖關節置換術分為兩個部分:上方盤骨的髖臼和下方的股骨,前者涉及把臼杯放置於原來正確的位置,就技術而言難度較高,尤其當處理複雜的髖關節疾病個案時,如患者髖關節發育不良,一般已無法找到臼杯原來的位置;而機械臂則可以準確地將人工骹植入於合適的位置,術後X光亦可見人工骹植入的位置比傳統手術更準確。利用機械臂輔助髖關節置換,不僅有效提升準確度,亦能減少術中創傷,加快病人康復,讓他們更早可恢復關節的活動能力。
